Why you were selected
This survey is nothing to worry about. Everybody who receives care or support services from JRH Support is being asked to help inform us on how their care or support is being delivered. We are also asking service users’ family and friends, as well as JRH Support staff and any professionals involved in the care and support we provide to give us their feedback. John Huthwaite is the owner of JRH Support and is keen to hear your views. Participation is entirely voluntary.
Who should answer?
The appropriate service user questionnaire should be answered by the person who receives care or support from JRH Support. A family member, friend or carer can help you complete the survey but the answers should be yours not theirs. There are separate questionnaires for family/friends, JRH Support staff and professionals. Please let family, friends and professionals know that they can complete their respective questionnaires online at jrhsupport.co.uk on the ‘John’s Survey 2023’ page. Please complete the survey only once.
How long will the survey take?
The survey should take no more than 10 minutes to complete. You are welcome to answer as many or as few questions as you wish. If you have been contacted by email, please click the link in the email to take you to the appropriate survey. Alternatively, please go to jrhsupport.co.uk and click the “John’s Survey 2023’ link and look for the appropriate survey for the type of care or support you receive.
Confidentiality
Your answers will be confidential. Your care worker(s), social worker or care/support manager will not see your responses unless you have asked them to help you complete the survey. You can leave your name if you want to. It’s up to you. We don’t mind if you do or don’t.
How the process works
We have already contacted you to find out how you wanted to complete the survey (on paper, online or by telephone). Where we could not contact you, a paper copy will be sent in the post. Each questionnaire is completely anonymous with no identifying marks.
How to complete the questionnaire
You will be asked to tick boxes and sometimes to write your views. We would like to provide you with the best service we can and giving us your views helps us to achieve this goal. Once you have completed the questionnaire please click ‘submit’. Please submit the questionnaire by 28/02/23.
The results
The responses will be analysed by John and our compliance officer to evaluate the services we provide and to find out what changes and improvements need to be made to these services. Results will be published on our website without any identifying information about you.
